Patient Rights and Responsibilities

Respect                            

Patients have a right to considerate and respectful care. They have the right to be treated with utmost sensitivity to their needs and feelings, and are to be treated as equals.

Choice                              

Patients are expected to be active participants in their health care and follow an agreed upon treatment plan. Patients have the right to accept or refuse treatment to the extent permitted by law. Patients have the right to change their health care provider if they are unsatisfied with the care and services provided to them.

Service                  

Patients have the right to receive efficient service, regardless of sex, religion, race, education, social background and/or economic status.

Communication                

Patients have the right to clear explanations of all information regarding their health care. Patients with limited English have the right to an interpreter if necessary.

Confidentiality      

Pursuant to the Privacy Act of 1974 and the Health Insurance Portability and Accountability Act (HIPAA) enacted in 1996, all information regarding a patient is deemed confidential and will not be released without written consent of the patient, except when required by law.

Privacy                  

Patients have the right to privacy. Those not directly involved with the patient’s care must have permission to be present and/or informed.

Information            

Patients have the right, to the degree known, to be advised of the diagnosis, treatment, prognosis, and any other significant information regarding their health. As such, the patient is responsible, to the best of their ability, for providing complete and accurate information regarding their health history to our providers.

Extended Care                  

Patients have the right to care beyond our capabilities. They have the right to be referred to specialists, treatment, and/or other services as deemed necessary by his/her providers

Comments            

Patients have a right to provide feedback and/or file a complaint regarding treatment at SYTHC through a grievance form available in the front office. Patients also have the right to receive fair review of feedback and/or grievances.

Civility                    

Patients are expected to treat staff and other patients/visitors of the Santa Ynez Tribal Health Clinic with respect, kindness, and cooperation. Patients are responsible for their own actions and behaviors.
